Here is a brief list of the most major insurances that we accept.  (If yours is not listed, please call our office for more details.)   It is your responsibility to contact your insurance company to verify your eligibilty before services are rendered.  All copays and deductibles are due at the time of service.

       Blue Cross/Blue Shield - Exam only  (Material coverage is usually a reimbursement from Blue Cross directly to patient)

       Cole Vision - Exam and Materials

       John Deere Health (Premier, Choice, Secure Care, and Secure Care Plus) - Exam and Materials

       Medicare - Exam only (A portion of materials is covered after cataract surgery only)

       Spectera - Exam and Materials

       Superior Vision - Materials only
